CRIS Healthy-Aging Center is seeking a Bachelor Level Case Manager. The Case Manager will provide services to seniors and adults with disabilities in Vermilion County. Must be driven, caring, and empathetic

Qualifications & Skills:

  • Bachelor’s Degree required preferably Social Service, Healthcare, Criminal Justice or related field
  • Self-starter & organized
  • Strong written and verbal communication skills
  • Excellent computer skills
  • Valid Driver’s License and reliable transportation with vehicle insurance

 

Salary will commensurate with experience. As a full time employee, you will earn 17 vacation days, 6 sick days and receive 14 paid holidays per year. Breakfast Site Manager

CRIS Healthy-Aging Center is looking for part-time Senior Breakfast Site Manager.  Ideal candidate will have food preparation and restaurant experience, preference given to candidate with Food Sanitation license. Hours start at 6:00a.m., 5 days a week and average 25 hours per week. Must be able to complete the Food Sanitation Course and receive Illinois Department of Public Health certification within 90 days of employment.  Manager will provide direction to kitchen staff in preparation of a full breakfast menu and in packing and delivering food for the “Meals on Wheels” deliveries.  Background check required. Bridge Transition Coach

CRIS Healthy-Aging is revolutionizing the world of transitional care management and we want you to be a vital part of our team as a Bridge Transition Coach.

A Bridge Transition Coach at CRIS Healthy-Aging coordinates care for patients by providing an essential service which helps the patient gain insight and access to services to help them achieve their highest potentials while recovering from a hospitalization. Your role is to support and assist patients in obtaining the resources they need to improve upon their health, happiness, and longevity. Bridge Transition Coaches are patient advocates that form ongoing, collaborative relationships with patients and providers to help improve a person’s lifestyle for the better.

From our clinical offices located within the hospital setting, you will spend a majority of your time speaking with and relating to patients over the phone to garner a more comprehensive understanding of their needs and concerns, work with patients so they can achieve their healthcare goals, update their medical records, assist with medication adherence, schedule future appointments, and so much more.

You will have the opportunity to meet with patients and family members in person prior to their discharge from the hospital, and will make home visits to address needs or concerns that require your in-person expertise.

What we are looking for in a candidate:

  • Excellent verbal and written communication skills
  • A desire to learn and a positive attitude
  • Previous experience in customer service, social services, and/or healthcare
  • Competency in typing and data entry
  • Valid driver’s license with reliable transportation

Preferred but not required:

  • Master’s Degree in Social Work or related field
  • 3 years of experience in social work, social services, or case management

You must successfully pass a background check and all occupational health employment physical requirements.

Senior Breakfast Site Assistant / Cook

CRIS Healthy-Aging Center is looking for part-time Senior Breakfast Site Assistant / Cook.

Ideal candidate will have restaurant experience and will require obtaining an Illinois Food Service Sanitation license within 90 days.

Assistant will help kitchen staff in preparation of a full breakfast menu and in packing food for the “Meals on Wheels” deliveries.

Hours starting at 6:00 a.m.,  2 – 3 days per week,averaging 10 – 15 hours per week. $11.00/hour. Background check required. Must have reliable transportation.
