Work Hours: 35 hour work week with rotating hours between 7:30AM – 5:00PM Monday – Friday: with once monthly weekend call rotation

CRIS Healthy-Aging is leading the movement of aging mastery and modernizing service delivery to our ever-growing older adult population. We want you to be a vital part of our team as a Community Care Unit (CCU) Case Manager. Our CCU department is the entry point for access to essential services for older adults who are aging at home within our communities.

A CCU Case Manager at CRIS Healthy-Aging provides broad-spectrum care coordination by linking our local elders with the vital programs and services they need to achieve their highest levels of success and functioning. Your role is to support and assist clients by developing an individualized care plan with goals to obtain the resources the individual needs to improve upon their physical, social, and financial wellbeing. CCU Case Managers are client advocates that form ongoing, collaborative relationships with clients and local community service providers to help improve a person’s lifestyle for the better.

From our office located within Vermilion County, you will spend a majority of your time speaking with and connecting to clients within the home setting to garner a more comprehensive understanding of their needs and concerns and coordination fundamental services and resources needed for clients to be successful in reaching their highest level of functioning.

Bridge Transitional Care Program Coordinator (Champaign & Vermilion Counties)

Description:

Our Leadership department at CRIS Healthy-Aging Center is seeking a social work professional to join our team as Bridge Transitional Care Program Coordinator. We are seeking a candidate with a Master’s of Social Work and 5-7 years of experience in social work case management, with experience in a medical field (hospital, hospice, home health care, pharmacy, ECF, or related). Additionally, supervisory experience of 1+ year(s) is preferred. Our Bridge Transitional Care Program Coordinator oversees many of the daily operations, staffing, and development for our social work model transitional care programs embedded at multiple local medical facilities. The prospective candidate will have mastered case management skills and be a self-starter who takes initiative in providing feedback and suggestions for programmatic development while nurturing new and ongoing relationships with community partners. Travel to and participation in occasional evening/weekend/overnight special events and training is expected a few times a year. Candidates who are outgoing, creative, and have a talent for public speaking are encouraged to apply. The hired candidate will exhibit personal traits that include promoting cultural diversity and sensitivity, empathy, and acceptance of one’s right to self-determination and personal choice.

Essential Duties:

  • Travel of up to 50-75%, primarily within 40-50 mile radius of Champaign/Vermilion Counties
  • Moderate to frequent walking of varying distances and/or periods of prolonged sitting
  • Flexibility to and implement progressive programmatic development
  • Assist with, manage, and audit caseloads and complete home visits
  • Collect, analyze, and interpret program data and assist in preparing presentation materials
  • Achieve and maintain proficiency in multiple computer software applications and electronic medical records systems with an ability to function concurrently within the applications
  • Maintain and update programmatic documentation including all forms, references, resource lists, and training manuals
  • Be a beacon for staff support and development, an infectiously positive attitude is key!

Work days/hours: Mondays – Fridays 8:00AM – 4:00PM, 35hrs per week plus occasional coverage for weekend/evening/overnight/special events.

Additional Requirements:

  • Current and valid driver’s license
  • Access to reliable transportation each working day

A Bridge Transition Coach at CRIS Healthy-Aging coordinates care for patients by providing an essential service which helps the patient gain insight and access to services to help them achieve their highest potentials while recovering from a hospitalization. Your role is to support and assist patients in obtaining the resources they need to improve upon their health, happiness, and longevity. Bridge Transition Coaches are patient advocates that form ongoing, collaborative relationships with patients and providers to help improve a person’s lifestyle for the better.

From our clinical offices located within the hospital setting, you will spend a majority of your time speaking with and relating to patients over the phone to garner a more comprehensive understanding of their needs and concerns, work with patients so they can achieve their healthcare goals, update their medical records, assist with medication adherence, schedule future appointments, and so much more.

You will have the opportunity to meet with patients and family members in person prior to their discharge from the hospital, and will make home visits to address needs or concerns that require your in-person expertise.

Senior Breakfast Site Assistant / Cook

CRIS Healthy-Aging Center is looking for part-time Senior Breakfast Site Assistant / Cook.

Ideal candidate will have restaurant experience and will require obtaining an Illinois Food Service Sanitation license within 90 days.

Assistant will help kitchen staff in preparation of a full breakfast menu and in packing food for the “Meals on Wheels” deliveries.

Hours starting at 6:00 a.m.,  2 – 3 days per week,averaging 10 – 15 hours per week. $11.00/hour. Background check required. Must have reliable transportation.
